Neurofeedbacktherapie bei Kindern mit ADHS: Prädiktoren für den Erwerb kortikaler Selbstkontrolle und die klinische Verbesserung

Die vorliegende Arbeit wurde im Rahmen einer umfassenden Studie „Neurofeedbacktherapie bei Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS)“ durchgeführt. Ziel des Projektes war, eine psychophysiologische Behandlungsmethode (Selbstregulation langsamer Potenziale bzw. des Theta/Beta-Quotienten) bei Kindern mit ADHS zu entwickeln. In bislang erfolgten Auswertungen der Studie konnte gezeigt werden, dass Kinder mit ADHS von beiden Feedbackverfahren profitieren. Da jedoch nicht jedes Kind gleich gut auf sie anspricht und der zeitliche und finanzielle Aufwand einer solchen Behandlung hoch ist, ist die Bestimmung von Prädiktoren für den Therapieerfolg von großer Bedeutung. Ziel der vorliegenden Arbeit lag demnach darin, Prädiktoren einerseits für das Erlernen der kortikalen Selbstkontrolle und andererseits für den klinischen Erfolg einer Feedbacktherapie der langsamen Potenziale und des Theta/Beta-Quotienten bei ADHS-Kindern zu ermitteln. 46 Kinder mit ADHS gemäß DSM-IV im Alter von 8 bis 13 Jahren nahmen an der Studie teil, von denen 25 die Selbstregulation ihrer langsamen Potenziale erlernen, während 21 ihren Theta/Beta-Quotienten beeinflussen sollten. Das Training bestand aus drei Phasen, mit jeweils 10 Sitzungen. Vor dem Training erhobene demographische, krankheitsspezifische, kognitive Maße, sowie Erziehungsmerkmale dienten neben der Diagnostik auch als potenzielle Prädiktorvariablen. Sowohl direkt nach der Therapie (Post 1), als auch ein halbes Jahr nach Beendigung des Trainings (Post 2) wurden erneut Fragebögen ausgegeben und Tests durchgeführt, um den Therapieerfolg und seine Stabilität zu beurteilen. In der Theta/Beta-Gruppe erwiesen sich körperliches und psychisches Wohlfühlen und eine geringere Ausprägung der Störung fördernd für die Verbesserung des Verhaltens zuhause. Gute Leistungen im Intelligenztest (HAWIK III) sprachen sowohl für die klinische Verbesserung, als auch für den Erwerb der Selbstkontrolle über den Theta/Beta-Quotienten. Was die LP-Gruppe betrifft, so war eine gute Anfangsleistung in der Negativierung der langsamen Potenziale, und eine gering ausgeprägte Symptomatik, sowie das männliche Geschlecht fördernd für den Erwerb kortikaler Selbstregulation. Für die klinische Verbesserung erwies sich, die Fähigkeit ohne Rückmeldung zu negativieren (Transfer) als stärkster Prädiktor, was für die Spezifität des Neurofeedbacktrainings spricht. Mit der vorliegenden Pilotstudie wurde Neuland betreten. Erstmals wurde versucht für zwei verschiedene Neurofeedbackverfahren Prädiktoren zu finden. Die hier ermittelten Ergebnisse sind eine Grundlage dafür, dass in weiteren Studien an einer grösseren Probandenzahl hypothesengeleitet Prädiktoren für eine erfolgreiche Neurofeedbacktherapie bei ADHS-Kindern ermittelt werden können.

Neurofeedback for children with Attention Deficit/ Hyperactivity Disorder (ADHD): Predictors for the acquisition of cortical self regulation and for the clinical outcome

This study was part of the comprehensive study „Neurofeedback for Attention Deficit /Hyperactivity Disorder (ADHD)“. The goal of this study was to compare two neurofeedback protocols: self regulation of slow cortical potentials (SCPs) and of the theta/beta-quotient (QT) for children with ADHD. Data analysis showed that children with ADHD improve in the target behaviour as well as in other behavioural variables and in cognitive functioning. As not every child derives the same benefit and because the treatment requires a lot of personal and economic efforts, the identification of predictors is the goal of this study. Methods: Forty-six children aged 8 to 13 who met DSM-IV criteria for ADHD or ADD joined the study. Twenty-five of them received feedback of SCPs and twenty-one received feedback of the QT. The training program consisted of 3 phases with 10 sessions each. Before the training educational features, demographic and cognitive parameters as well as symptoms of ADHD were assessed. Measurements were repeated after the end of the training (post 1) and six months later (follow-up). Results: In the QT-group satisfaction of children with their physical and emotional conditions as well as a small degree of the disorder predicted improvement of the behaviour at home. A good performance in the intelligence test (HAWIK III) predicted both good clinical outcome and the acquisition of self control of the QT. For the SCP-group a good performance in the negativation task of the SCP-training at the beginning of the training, a small degree of the disorder and male sex predicted the acquisition of cortical self control. The ability to negativate without feedback (transfer trials) at the end of the training was the strongest predictor for the clinical improvement. This seems to indicate the specifity of the clinical effects of the training. Conclusion: For the first time predictors for the acquisition of cortical self regulation and for the clinical outcome were assessed for two different neurofeedback protocols. On the basis of the ascertained results further studies can investigate predictors for a successful neurofeedback therapy in children with ADHD on a larger amount of examinated subjects. These should extend the criteria of success in both quality and quantity. In addition predictors should not only be determined for the success of the therapy, but also for the stability of this success.
